    It sounds like an input voltage transient.  You may need an Al. input cap to help damp the input voltage transient.

    How are you applying the input voltage for the test?  Are you using a fast switch? Or "hot plugging"?

    Can you monitor the input transient?  How high does it go?  Can you send a scope shot of the input voltage during the test ?

    What is the damage?  Is anything else damaged?  Is the Schottky OK?

    Can you send a snap shot of you layout?
