TPS59641 Power Good Question

My customer is seeing the following issue with the Power God pin output of the TPS59641:

I have a question regarding the TPS59641RSLT.  We have started field testing with a customer on our product using the TPS59641RSLT.  We have a failure returned from the field.  Below are traces of the power good from the TPS59641RSLT.  From the datasheet, if an over voltage, under voltage or over current event occurred on the output, I would expect the power good to latch low and the output of the regulator turn off until VR_ON is toggled.  We are seeing oscillation on the power good.  Does this indicate a under voltage on V5 or V3R3?  Attached are a few traces of the power good.  Do the on / off times align with any internal timings of the TPS59641RSLT.


Should the output be oscillating at this ~50% duty fixed frequency (~250 Hz)? I do not have a copy of the data sheet so I do not know of any ~250 Hz frequency that would be available inside the TPS59641. Can you explain what might be happening?
