Failure of PWM output, pin 19, on UCC2305N. Lower device leaking

Have experienced an increase in the number of PIN 19, PWM Output signal from the UCC2305N.   I had some failures decaped and photographs taken.

I was hoping to find out if it was the lower CMOS device or the ESD diode that was defective.  Then looking at the picture of the die, the intrinsic diode of the N channel MOSFET was what I thought was the protection diode.


The failures are very random.  I am trying to figure out if I should be looking for an over current or reverse voltage transient.  At first the IC evaluator said they could not see the damage. I have photos but do not know how to interpret the results.  Since the damage looks so minor in the photograph I am assuming this is being caused by a very narrow transient.
