I am using the TLV62130 in the following circuit to generate +3.3V with a +12V input supply. The current required at +3.3V is about 500mA. I have had 3 TLV62130’s fail after several hours of operation. I have followed the printed circuit board guidelines for thermal dissipation with the bottom ground pad soldered to the PCB ground plane.
Inductor L1 is a Bourns SRN8040-2R2Y. Capacitor C19 is a Kemet T521V226M025ATE060.
Everything looks correct according to the data sheet. Do you have any idea why these parts are failing?
