I like to use TI's Webench Designer for ideas.
Since this is an isolated output, you can connect it as either positive or negative.
Entering 12Vin, 40Vout at 0.3Amps, and Isolated Output, provides 14 designs.
An isolated 40V output can be connected as a negative output.
Attached is the design report using the LM5160A.
The transformer is noted as "custom" but you could find some off the shelf part that would work from one of the major transformer vendors.
