TPS54260: Reg Issue with TPS54260 device

Dear All,

one of my customer working on Dual USB charger based on TPS54260 device, 

I have attached the schematics  of dual charger for 48V input output 5v@2.5A.

Till now it is converting 48V to 5v at no load. When we put the load first voltage start decrease for 5.10V to 4.50v @0.6A .if we continuously increase the load at 0.8A the IC stop working ( showing input short ) . In the IC side VCC and GND showing short . kindly advise your valuable feedback.

    Can you check your supply current limit level? It sounds like the power supply has very low current limit. When you pull more current out of the supply, the supply voltage will drop.

    If the supply current limit is not the problem, please capture waveforms of VIN, Vout, PH pin and SS pin to help identify the root cause. Please capture waveform when there's no load, 0.6A and 0.8A.
