lm5175: low Vcc and heat issue

Hi.

I have the problem with LM5175P.
I designed my scheme very close to the variant offered by Webench. Requed parametres were: input 10-18V, output is 12V and 3A max (not constant). I'm ready to send my scheme in *.pdf format at e-mail you'll specify.
In a few seconds after the power supplied LM5175 starts heating quickly. I swich off power supplay after 5-7 sec to prevent chip's overheating (I can't keep my finger at the chip anymore at this time).

The test conditions was:
Vin = 18V
Vbias pin is not connected to output
Ven = 2.34V
Rload = 12 Ohm

Vout = 12.3V
FB = 0.85V
I measured incorrect Vcc voltage: Vcc = 4.7V (instead of 6.95V min)
Voltage drop at 10 Ohm resistor at Vin pin is 0.783V (thus Ivin = 78.3 mA instead of 4mA max)

It seemed to me that something is wrong with EN & BIAS LOGIC inside this part.

Can I do something to fix the problem?
