LM25011: No output from the LM25011

Here is schematic. Layout to follow. 

Bootcap and CS/CSG connections all look solid.  Below is the layout. I could only capture the top layer, but I put in notes that hopefully clarify.

I'll see what I can provide as far as probe images on the nodes requested.

There seems to be multiple issue in the layout of the design . The two major current loops conduct currents which switch very fast, and therefore those loops must be as small as possible to minimize conducted and radiated EMI. The first loop is formed by CIN, through the VIN to SW pins, LIND, COUT, and back to CIN. The second current loop is formed by current sense RS,  diode , LIND, COUT, and back to RS. The ground connection from CSG to the ground end of CIN should be as short and direct as possible.

You may use EVM as a reference for your Layout .(Refer to Small bypass capacitor CIN etc) 

Further in your design , you could have easily GND the copper pour in this layer . The layout is a major factor but if the connections are right it should switch and give us valid output . 

Can you please ensure that the exposed pad(power Pad is connected to GND) .

I have completely rebuilt this circuit on a breadboard and it still doesn't work.  I think there is something fundamentally wrong with either the part or with WebBench. I think I am just going to select a different part and abandon this thing.  Wasted too much time already on something that should just plug and play. Thanks for your time.

When you breadboard the circuit , how are you ensuring that the power pad is connected the GND ?One Easier way to evaluate would be 
 order an EVM and change the circuit as per new design requirement .
