the output is around 2.3V, it should be around 15V, please see the schematic below, why it happen?

Make sure your power is not current limit.
Then upload the waveform of the SW and you PCB layout

D2 GND node should connect to C175, C176 GND as close as possible. you can temporally solder D2 between PH node and C175 GND to see if any improvement.
