TPS61021A: Customer has had multiple failures upon power up

HI Support team

My customer has had multiple failures in their prototype testing phase, here is what they said:

We are testing prototypes with the TPS61021 regulator.  We have had 2 unit failures during the course of our testing. 

 

The failure mode results in the TPS61021  SW pins being shorted to ground.  The EN, VOUT and FB pins are not shorted to ground during this failure mode.

The test conditions involved powering up the TPS61021 with VIN = 1.52VDC and output set for 3.3VDC.  The failure modes occurred at power on.

We would like to protect the TPS61021 to prevent future failures. 

Can you help us identify what might cause a failure?

[1] Could this be an ESD related failure?

[2] Could this be due to too much input current into the TPS61021?  I am sort of expecting that the internal transistor is shorted.

[3] Are there any recommended ways to protect the TPS61021 from this shorting failure?

I will email you schematic and also some plots along with more info on the use case.
