LP8758-E0: Issues with one buck regulator out of four

I'm having a little trouble bringing up buck3 on my regulator.  I built this design on another board and all four worked fine.  Now however on two new boards buck3 does not work.  I leave it set to the default 1.8V and enable it but I get a steady 560mV output.  I don't observe any switching on the near side of the inductor.  I checked the status registers and it tells me that the regulator is enabled, but under voltage.   The part is cool under a thermal camera.  Input voltage is 4.2V.

I'm sure it's something we've done but I thought maybe this steady output voltage might be a clue you could help me with.
