In order for us to be able to help you debug your application and understand what is going on, we need more information. Please provide a description of the issue and scope shots of the issue. The scope shots should include Vin, Vbias, Vout, and Iout.

You mention that you are following the recommended circuit, could you confirm if you are referring to the Typical Application Circuit on the front page of the datasheet?

Vin=3.3V, 

Vbias=3.3V

Vout= 1.2V

Iout= 10mA

As i explained in my earlier post we are facing failure of LP38858S-1.2.  Outputs are not coming we want to know the reason for failure

Your LP38858 in operation, becomes it hot?

How high is the total load capacitance at output of LP38858?

How high is the output current?

Can you show us the layout of your LP38858?

1) 1N4148 ID OK

2) LP38858 when in operation it is not hot

3) Output capacitance is 10uF

4) Output current drawn is MAx 10mA

the LP38858 is not becoming hot? Hhm... Last try: Do you exceed the maximum input voltage range? Is there a chance that the input voltage exceeds 5.5V? Or are there any overvoltage spikes exceeding 5.5V? How do you power the LP38858?
