The most probable cause would be an ESD strike. You could send a couple of devices for failure analysis back to TI, they may be able to locate where the electrical over stress happened and if there is any other failure on the device.

We also have a number of failed BQ34Z100-G1 Coulomb Counters, we believe because the OEM supplier hasn't taken sufficient ESD precautions. We need assistance to analyse the exact cause of failure to know where to add further protection, and to support our request to our OEM to re-design the product accordingly.
