TPS61088: Enable pin connection, TPS61088 broken

Good day, 

I'm using TPS61088 and I have connected the EN pin to VIN that is 5 V (less than max 7 V). Two TPS61088 devices have broken down without excessive load at power-up and I'm wondering if my EN connection is the reason. I'm boosting 5 V to 12 V and with these broken devices the output voltage is bit less than 5 V (instead of 12 V).

Can the device get broken if EN is connected to VIN (is it allowed that VIN and EN rise simultaneously)?

VCC rises a bit later than VIN. Is it possible / recommended to connect EN pin to VCC (VCC voltage is 5.8 V)?

If connection to VCC is not recommended, does TI have reference connection for TPS61088 automatic startup?
