LM3478QMMX/NOPB

I am seeing quite a few failures of my circuit and can't seem to see what I am doing wrong!  if an expert can take a look and see if there is an issue, it would be appreciated!  Some details on the circuit:

Vin is 24V or 12V, but mostly will be at 24V.  Output is 12V. the load is an SBC which says ~14W max.  so only an amp or so on start up. usually around 400ma.  Q6 appears to keep blowing in my schematic, which is the FET from the DR pin of the switcher.  What am I doing wrong?  please help.  thanks!  circuit below
