The layout is not good, did you test waveform on SW node?

Issue resolved D1 was wrongly soldered ,

Hi Gaurav,

bellow some suggestions:
1. Move C29 up
2. Place D1 on C29 position
3. Rotate C28 left 180 degree
4. Use more thick trace between U3 SW pin and inductor, D1
