TPS65217: Active Power on L1, L2, L3 to ARM upon AC power

I have a number of BeagleBone/BeagleCore modules (which used this TPS65217 PMIC) have gone bad. Not sure why TPS are so fragile.

I am assuming there was an overvoltage (>5.8V but under 17 VAC) maybe due to turn on transient (even operating on low 18V-5V DC-DC supply) but the PMIC protection should shut down without damaging the TI ARM Cortex. But I believe the TPS65217 on all the boards have stopped working. 

I wanted to confirm that the TPS should go from OFF or SLEEP state to POWER ON (active) state at L1, L2, L3 outputs when powered at AC pin for at least 5 secs even without any external components or PWR_EN, PB_IN, nWAKEUP handshaking required.
