LM26420: LM26420XMH/NOPB failure

We're seeing a fairly consistent failure with the LM26420. We've been using this part in many designs for years, so are finding this surprising. We are finding that on some devices, the low side FET on output 2 is breaking down after multiple power ups. We are detecting this by isolating the output and measuring the output to GND. The initial resistance is multiple Ks to GND, when we see this breakdown, the output resistance to GND drops to a few Ohms then sub-Ohms. The part gets incredible hot at this point and obviously can't regulate.

Has this part had a recent die change? Any idea what could cause this failure? We're trying the same test on a different PCB to see if that also fails.
