TPS561208: TPS561208 destroyed after power up

Hello,
I just power a new custom board, the two TPS561208 on it created a shortcut.
We unsolder them.
The first one have GND and SW which are internally solder together.
The second one have GND ,SW and VIN which are internally solder together.

We have other component powered by the same input which are working fine :

TPS53513 and TPS563219DDF
We are wondering what could cause such effect.

In the attach schematic Vin = 12V.

Hi,

What is the power supply for 12V? Is there some ringing on the input voltage? Please try to power TPS561208 by using external 12V power supply to check whether the device would damage.

Thank you for your fast reply,
The 12 V is generated by a LM5176PWP and the voltage is well regulated without the two TPS561208. I ll resolder new one and bypass the LM5176PWP with a external 12V power supply.

Hello,
I have solder new TPS561208 and and power them with an external 12V power supply. I get the same results :
Both of TPS561208 GND and SW are internally solder together after power up.

I removed the L_filter_1_8V1 and L_filter_2_5V1 to isolate the regulators from the rest of the circuit, I changed the TPS561208 and power them with an external 12V power supply.
Both of TPS561208 GND and SW are internally solder together after power up.

I'm checking the PCB again, if i get an update, I ll let you know.
