TLV62569: Both High-side and Low-side MOSFET Short Issue

Hi, Team

My TV customer is using TLV62569 in their TV board to provide core power of processor RK3128H.

The input voltage is 5V, and output voltage is 0.954V ~ 1.395V. CPU controls the output voltage by 200kHz PWM signal.

Customer manufactuer 200 units and find 1 units failure. Both high-side and low-side MOSFET of TLV62569 are shorted.

Here is customer schematic. Could you help to check whether there is any issue? What is the posible cause?

    Having a short on both MOSFETs indicates an EOS event. We need to find out were this comes from.
    Regarding the schematic, I would recommend to add more output capacitance (2x22uF) as recommended in table 4 of the datasheet
    Could you share the layout for reviewing?

    Usually a spike in the VIN or SW pin could cause this type of issues. Could you share the layout for reviewing? Could you precise where they found the failure? (in-circuit test, functional test, production test...etc)
