CCS/UCD3138: The device gets no responding to PMbus after downloaded firmware everytime.

Tool/software: Code Composer Studio

After I downloaded th firmware, next time to download , the UCD3xxx Device GUI will gets as follow, indiating the device broken.

Is the firmware downloaded from ti.com?
Have you modified this code?
Do you have a bootloader in your code as well?
Please advise.
