what woule happen if VOUT = 4V and VIN =1-2V?
will there be reverse current flowing?
is there any risk to damage the part?

Thanks for reaching out on E2E!
The TPS22917 has reverse current blocking functionality when the device is either enabled or disabled, hence the "always-on RCB". Even if VIN = 1V and VOUT = 4V, the I_IN,RCB (reverse leakage) will end up being less than 1uA.
