As posted by Andrew Page in this post, we are also facing the exact same issue of failure of BQ34Z110. After the failure we see that the REGIN shorts to ground and as a cascade effect the voltage regulator (TPS73033DBV) fails due to over current. We have been using BQ34Z110 for the past three years and this issue has surfaced in the last production batch only.
