Thanks for reaching out on E2E!
It is difficult to predict the failure mode of the ULN2003A due to an over-voltage or over-current event. Since the FET could be stressed causing the channel resistance to increase, the FET could fail open or create a short from collector to GND.
If you could provide more details about the application, I can take a closer look and see if there's anything wrong with the schematics / application.
