I would not recommend trying to modify PMP10523 to meat this design. The major reason is the that power level is very very large (~1.44kW) and would be better suited for a multi-phase design or an isolated solution like an active clamp forward.

What is the application that requires this much power? This information might help to point you in the correct direction.
