    Hi Eddie,

    Please find my comments below:

    Question1-2a - No, please ensure that the ratio of the two dividers to the Vsense and  Vinac pin are identical. Please read section 7.3.6 of the datasheet for further information. 

    I would also point out that the 470nF of filter cap across RB of  Vsense is too large; please make sure that the RC time constant do not exceed 100usec. Also explained in detail in 7.3.6 section of the datasheet. 

    The waveforms look alright to me.
