For a 12V output Class 3 design, please refer to PMP4679 which uses the TPS23750 PoE PD. Please let me know if this is not sufficient or if you have any additional questions.

Frank, The best way to select a PD+DC/DC PWM is to go to the product selection tool and then click on "DC/DC Control" --> "PD and DC/DC". That will result in 11 possible matches. Most of those 11 matches will be overkill for a Type 1 limited design. I would eliminate any of the options which exceed the power level required (13W and below) and focus on these options:
TPS23750, 23770, 23757: First generation PD+PWM controllers; higher prices; wide Vin
TPS23753A: Workhorse of the Type 1 PD+PWM controllers; gen. 2; lower IC price; a lot of reference designs to choose from
TPS23755/23758: NEWEST Type 1 PD+PWM controller family; highest integration; lowest overall system cost
