For the bad modules, is the output shorted to ground? If you replaced the part with a brand new module, are you getting a stable 1.2V output voltage? I will spend some time looking over your schematic and give you some feedback on this.

Perhaps the module had an input voltage overstress that damaged the part. You should try to measure the resistance and voltage on pin 7 with power applied and no resistor to ground. The maximum voltage out of pin 7 should be 0.7V when open. Anything higher than that when open could signify that the controller has experienced some damage.

Customer test normal module pin7 foot-to-ground voltage is 0.61V, problem module pin7 foot-to-ground voltage is 0.02V.

The bad module sounds like the part is no longer internally regulating. This would be how I would check between a normal working power module and a "problem" module. If the voltage is too low like in the problem module's case of 0.02V, it can be infered that the internal part is damaged. Is this the only damaged part or are there others? Can you also probe their pin 7 to ground voltage?
