TPS40200: No-Vout and oscillation frequency error

Hello E2E,

Please let me know about this device.
One day, end equipment used TPS40200 has damaged in during normally operating. That end equipment was shipouted from our customer in two yers ago.

SS-pin voltage rise up to over 8V, reach to about 13V.
I found for folloing post.

I think that content of this post likes to our case.

Questions;
  -  Have previous posts been resolved?
  -  TPS40200 has voltage clamp circuit for SS-pin. Does TI know why SS-pin voltage rise up to above 13V?
  -  Does Vsst internally regulator have damaged? What is cause?

    What's the input voltage range in your application? what's output voltage and load current? 

    can you upload the circuit of TPS40200 section for checking?

    Pls try to contact TI local distributor to request failure analysis if TPS40200 was damaged or output was unregulated.

    Vin = 24V

    Vout = 12V

    Iout = 2A_max

    We can't upload the circuit by our policy because it is customer information. If you let me know your e-mail, I can send the circuit schematic you.
