TPS61165: LED Driver fails after a few days of usage (Vout = Vin)

Hi Team,

My customer is having issues with the TPS61165 LED driver. On 20% of their devices, they are seeing that after a few days the LED driver stops powering their LEDs. In these cases, Vout = Vin. The specs for their display are 16V, 40mA. PWM voltage waveform is 3.3V at 30kHz. They have tried testing the LEDs with an external source to verify that there is no issue with the LEDs. 

Do you know what could be causing this issue?
