UCC28950: UCC28950 failure

I use UCC28950PW in a battery charger, tipology fase-shift with sincronous rectifier.
I have had number 3 breakings (2 from customers and 1 in Deca laboratory). All over 3 breakings have had the the same failure: OUTB (pin 21) & OUTC (pin 20) shortcircuited to GND (pin 24).

I'd like to have some technical information about it.
I could send electrical scheme if is necessary.

Hi Leonardi,

The B/C driver output absolute Maximum Ratings is from -0.4V to VDD + 0.4v. you need to check the voltage stress. in ordering to avoid high voltage stress, the layout have to avoid long and narrow trace to avoid Parasitic inductance. so, good layout is important.
