BQ25895: BQ25895 failure analysis

Hi team,

Customer is using our BQ25895, and they are meeting failure on some of there product, the Failure phenomenon are as below:

① When PCBA is not powered on, the DC impedance of Vbat pin and VSYS pin is only about 5Ω;
② When VBAT supplies 4V using Keithley, vsys voltage is about 3.26V, and vbat has 240mA current output.
③ The DC impedance of Vbat and GND is 194Ω;
④ When plugged into USB for charging, Vbat is set to 4V. At this time, the charger status indicator is always on, and the VSYS voltage is 4.23V. At this time, VBAT does not charge but outputs 65mA current.

The following is the schematic, could you give some comments on this?
