Some advice for your information:

1: C19, C20 are not needed

2: C17, C18 should have total equivalent capacitance about 0.1uF

3: 2 100R resistor is needed between D7, D6 and SMBC, SMBD for better overvoltage and ESD protection

4: R3 and R4 should be greater than 2k(5k preferred) considering a -20 negative charger applied at Pack+ and Pack-

5: DS pin connection of Q12 should be swapped.

regarding point 5 - could you be more specific? You mean I should swap Drain and Source of Q12?
