BQ40Z60: HIDRV FET gate controller is malfunctioning.

I made a 4S1P battery configuration to perform charging and discharging. My battery packs total voltage is 14.7, I checked voltage at VSYS it showing almost same as battery output so, I consider discharging path is okay. But, when I supply 18V at adapter ACFET set to HIGH what I observed in Registers window is FET temperature is increasing (65+ degC). Then I measured voltage at VSYS it was 2.14. so I stared measuring all the voltage at gates and Drain of MOSFETs. Got results as shown below:

Q5 Vs = 17.915V, Vg = 29.758V, Vd =17.922V (Gate is connected to ACFET of BQ40Z60)

Q8 Vg = 2.14V, Vs = 2.14V   (Gate is connected to HIDRV of BQ40Z60)

Q9 Vg = 0.313V, Vd = 2.14V  (Gate is connected to LODRV of BQ40Z60)

 At this condition XCHG, XDSG, SS, OTA, SEC0, STH are set HIGH. I didn't find why it is happening, Please reply to me if you found any suggestion are loop holes to close this issue. (Jumpers and all connection were good even all the Hardware looks fine).
