BQ24295: BQ24295 changes itself from charge termination to fast-charging and to charge termination mode several times

FAQ #1 describes the recharging behavior of the IC. I have checked this abnormal transition from 11 to 10 to 11. It doesn't happen because it reached recharge threshold. This behavior happens at every 1~5 minutes. Each line in the example log, given in the original post is read every 5 minutes

Please provide your insights on the same.

What are your test conditions such as VBUS (voltage at VBUS pin), VBAT (voltage at BAT pin), IBAT, VSYS, ISYS? Could you please provide all register values of the charger? Here are the examples for register value formats: REG00=A4, REG01=84...
