LM5088-Q1: IC experiences issues during testing

Hello,

Client using LM5088-Q1 in his power supply project - Vin=13-70V, Vout=12V@3A.

During testing the IC & MOSFET burn down if Vin > 52V is applied. This happens with no load and 1A load.
What might be the cause? Could this be an issue with PCB layout or component selection?
Scheme is attached.

I would first start with quick start calculator on development tools tab.

Quick way of verifying appropriate componet selection. Please be sure to reference the BOM for populating calculator with correct component paramaters.

Next step if componet seleciton seems alright would be to evaluating testing method.
