Unfortunately, I cannot find a reference design with the LM3478 that meets your exact specifications.
However, I've attached is a LM3478 reference design that generates 200V @ 0.01A http://www.ti.com/lit/df/tidre38/tidre38.pdf?ts=1590514901543 that utilizes a boost converter followed by a charge pump. Unfortunately, a charge pump will make your output current much smaller than what you would need. Additionally, if you use a single-stage boost converter your duty cycle is quite high, and as such this will limit your switching frequency and selection of available controllers.
There is another reference design that follows a similar concept and utilizes LM3481 http://www.ti.com/tool/PMP20183.
You may try cascading different stages with different controllers to see if you can generate your desired output, but there seems to be no available reference design for this.
