LM5163: LM5163DDAR: 48Vdc input / 12V out fail problem

Hi Ti,

Attached file is my circuit. And my test condition is 48Vdc in / 12Vdc 0.02A output. After power on for 1~5 minutes, the IC burn out suddenly. I used infrared to monitor the package temperature of IC which was <40 °C, so I didn't solder the heatsink in the back of IC.Do you have any suggestion?
