TPS63802: Need assistance to identify causes for TPS63802DLAR IC burn out

Hi, 
I have designed a TPS63802DLAR Boost converter. 
Following image details the Input/Output parameters designed for. 
Have followed the same Schematic. 

This is the PCB layout for the Boost Converter 

Application - Powering a 5V Lock + Buzzer combination with a 3.7V Li Ion Battery input

The TPS63802DLAR  constantly keeps burning when connecting input power out even without connecting a load. I'm assuming the IC has burntout since inital tests after board assembly used to yield 5V output however after a while, the IC tends to heat up and stops providing 5V output. 
Is there anything I need to rectify in my PCB layout or schematic that could help solve this issue.
