TPS61322: TPS613222ADBVR Overheating and not working properly

I really need your help to try to get this boost converter working.  I am trying to use TPS613222A using a LiPo battery 3V to 4.12V to get 5V and 150mA max.

1.  The U3 chip below is running very hot.  There is no load at V_FAN TP10 yet, it is running hot.  I didn't expect that.  Temp at the chip package may well exceed 50C and it can burns your finger tips if you touch it.

2. What should the value of L1 be?  To try, I additionally bought 4.7uH to reduce the average current, and I will try C8 at 47uF to see if brings the operational temperature lower.

3.  What should the Snubber values be R16 and C10?  The TSS83L45 calls out a Junction capacitance of 220pF.  This Schottky diode is good for 3A.  I have tried up to 5 ohms and 1nF no difference.

4.  The output voltage never reaches 5V.  It duels around 4.3V etc... Something is not right.

Overall, if I put the scope at TP10, I see bursty mode where it is discontinuous.  I am not sure if this is because there is no load, or because thermally it is shutting down on/off to protect itself.  Overall, I didn't expect this behavior nor did I expect an extremely hot chip.  The battery with 4.12V has a small boost to get to 5V plus losses etc.. but it in my mind it didn't expect it to burn your fingerprints (LoL :) )
