Do the 5962-1222402V9A and 5962R1222403V9A use the same die? Is the only difference that device type 03 is upscreened and passes radiation characterization testing?

Michael,
That is correct. All of the -SP orderable devices use same die, just to different levels of lot acceptance.
Note, that they may not be from same die lot as the RHA version.

Thank you for your response. Looking at the DLA SMD spec, the 5962-1222402V9A is the TPS7A4501KGD-SP, which has the "SP" at the end and meets the requirement. However, the 5962R1222403V9A is the TPS7A4501RHAKGD, which does not have the "SP" at the end and does not meet the requirement. So the 5962R1222403V9A (TPS7A4501RHAKGD) does NOT have the same die inside as the 5962-1222402V9A (TPS7A4501KGD-SP). Do I have that correct?
Michael, I should have been clearer.
When I mention the -SP, I am referring to all the orderable devices on the TI.com TPS7A4501-SP product folder. TPS7A4501-SP is the generic name. All of the devices listed there are using the same die.
Hopefully this clears this up.

Wade
