BQ24296: bq24296 sys short to ground

Hi

My customer design schematic with bq24296RGER, produce with 10pcs sample board, 2pcs sample board work in boost mode but when the 5V adapter is inserted for charging at the first time, SYS is instantly broken down and short to ground. 

At this time, these 2pcs still can be charged, but the charging current is only about 110mA.

The other 8pcs sample board is work normal.

The customer want to know how to avoid this problem.
