Hi. I'm having a similar issue as explained on this ticket.
The only differences are:
Output voltage set to 12V
Frequency set to 800KHz
UVLO level set to 15V
All settings are based on Datasheet suggested values.

Can you please provide the schematic? I would like to make sure everything is okay.
I suspect the device might be going into overcurrent protection and it is repeating the inhibit cycle. See Overcurrent Protection Section for more details.
How much droop is at the input? Please provide Input voltage reading.

It's the exact design as the one in the datasheet. Output current does not exceed 1.4A. I have confirmed it via a current probe.
There is also no sharp current spike or noise on the 12V output.
