TPS2024: Output of the Switch getting Short

Dear Member,

We are using TPS2024 as a USB power switch. The circuit diagram is attached below.

We are using this circuit in all of our designs. There are no issues observed till now. But, in our few recent products, the IC output is getting short during production testing.

The pin 6,7,8 is getting short w.r.t pin1 (typically impedance is 4-22 ohm). Only one modification is there it is regarding Power On Sequence(POR).

With the new design it is 5V-->3.3V, while with the old design it is 3.3V-->5V.

The input supply of 5V and 3.3V is having a tolerance of +/-0.1V.

Following are the waveform observed in the faulty chipset:

In the above figure, the Switch output signal should be the same as the switch input. But instead, the output is generating a square waveform. The IC is getting overheated during this functionality.

Can anyone suggest the cause for the damage that is occurring in the IC?
