Attached Schematic. The fault phenomenon is PH pin short ciruit with GND.
Our Vin 12V ,Vout 5.1V,Iout 2A .
We test about 60 pcs and failure about 11 pcs.
Could you help me know the reason,many thanks!

Thanks for your question. From the schematic you provided, I do not seen any reason that the output should short to GND. A few things to look at would be
1. Evaluate the LTxxx load with an external supply to verify that it is not causing the behavior.
2. Look at the layout carefully to ensure there are no solder bridges or shorts in the manufacturing. Since you only see the issue on some boards, this may be the case.
3. Verify that the output voltage divider is installed correctly.
