LM5116: LM5116 failure of high-side gate drive

I am investigating a string of failures of the LM5116 in a DC-DC buck module.  What types of abuse would cause the High Side Driver to fail low impedance HO to SW, leaving most of the rest of the chip operational?

The circuit provides a 24V 10A output from an unregulated 60V input. The same FETs and layout ares also used for a 48V 5A output (from 80V supply) which has seen no failures of this driver.

On the bench all chip inputs appear to be well within absolute max limits.
