BQ2057W: BQ2057WSN damage

Good morning,

I'm asking your help because I'm having problem with BQ2057WSN mounted on a PCB in order to charge a 2S1P Li-Ion battery, here the link with all technical information:

The power supply that I'm using to recharge the battery is a 12V medical degree, from MeanWell (model GSM60E12). The length cables is about 90 cm.

What I'm facing during the test of the PCB is that randomically, when I plug the power supply, the BQ2057 gets damage in an irreversible way.

I attach here the schematic:

Components highlighted in green are not soldered on the PCB.

The BQ2057 is configured in MOSFET configuration, with a recharge current of 500mA.

The p-MOS model is: 781-SUM55P06-19L-E3.

Do you have an idea of how I can remove this strange behavior?
