When customers use tps54360dda, they find that VIN, SW and GND are short circuited together. What is the general reason for this?

I assume the device becomes broken and results in a short between VIN, SW, and GND. Otherwise if the devices are tested from the tape/reel and are broken, please let me know.
Do any of the pins exceed their abs max ratings? I'd look mostly at VIN.

The customer added TVs tube at the input, but the TVS tube was not protected

Have they checked that VIN is not exceeding the absolute maximum rating?
