TPS7A16 / Locked to Disable

Hello there, 

There has been some interesting behavior seen on a couple of TPS7A1601DGN's. I'm wondering if you might be able to help get to the bottom of it.

Input: 3.45V from battery

Output: 2.5V @ ~100uA

Although the input and enable are tied together after the diode and 3.45V is seen on the enable, sometimes the device will enter a locked mode during which it will produce a .1V output instead of the expected 2.5V. The only way to kick it out of this is to disconnect and reconnect battery supply. Is this behavior something you have ever heard of before? What are some things that could be pushing the IC into a locked disabled state?
