We have tried to contact Ti FAE, but they suggest submitting E2E. Customer use IWR6843AOP to fall detection. Does the white area need to be completely hollowed out?

For the space constrained applications you could use the small formfactor from the TI EVM.

Void or cutout you pointed above helps to overcome small ripples in the antenna radiation patterns. In the some of the short range applications it may not affect the operation, Hence customers need to evaluate this.
